Fees & Insurance

Spark Providers currently accept Aetna, Blue Cross / Blue Shield, Cigna & United Healthcare for direct billing. 

 

Additionally, we are credentialed to accept most, but not ALL, Arizona & Colorado Medicaid plans, but we do require a pre-authorization before we can fully confirm your booked appointment.

 

All other insurance carriers will consider Spark as an “out of network” provider.

 

That said, even if we do not accept your insurance, most of our patients are able to get part or all of fees paid by insurance using typical “out of network” reimbursement options. 

 

Please verify with your insurance carrier if reimbursement is important to you prior to scheduling or paying for Spark Services.

 

Upon request, a formatted Insurance Claim Form with appropriate diagnostic information can be provided, so patients may submit their paid Claim Form directly to their insurance carriers for direct reimbursement.

 

Spark Fees or copays are prepaid prior to each appointment, and failure to pay within 24 hours of the appointment time may result in automatic cancellation of the appointment.

Medication Evaluation For New Patients

Visits typically 50 minutes by Televideo – $275

Follow Up Visits – Medication Management

Standard Follow Up | Medication Management – typically 20 mins by Televideo – $150

INSURANCE

Spark gladly accepts and bills most private Insurance & State Medicaid plans.
